Job description:
- Carries out, under the authority of their line manager and as part of a multidisciplinary team, a round of visits to multiple patients, providing direct care and assistance in their homes (support with hygiene-related care, help with daily living tasks, etc.).
- Provides all assistance outlined in the care plan, as well as any additional support aimed at promoting the patient’s physical and mental well-being.
- Using their general skills, they perform, promote, and ensure the highest quality of care and assistance.
- Through their open-mindedness, motivation, and professional commitment, they embody and promote the skills and values of the service to the patients.
- Contributes to the growth and success of the service, as well as to its positive reputation.
Profile required:
- Hold a certificate in social and family assistance ;
- Fluency in Luxembourgish and French is required;
- Professional experience in a hospital or outpatient setting is considered an asset.
- Possession of a valid B driving license
